What you get

Three things change in the first month

Not a subscription to more paperwork. A person who takes the medical side of your family's life and runs it.

A health advisor reviewing a care plan with a client
01

One clear plan

Every diagnosis, medication, provider, and appointment pulled out of five portals and into a single plan your whole family can read.

The Canyon Care advisory team
02

One named advisor

The same person every time, reachable on a direct line. Independent, with no financial stake in the providers we recommend.

03

Your hours back

We make the calls, sit in the appointments, and fight the denials. You get the summary and the rest of your week.

Who's actually handling it

The people your parent's care goes to

You're handing over something that matters. Here's exactly who picks it up, and why they can do it well.

Registered nurses

Clinical, not administrative

We read the chart and understand the why behind every order, medication, and follow-up. That's the difference between coordinating care and just filing it.

Decades bedside

We know the system from inside

Years spent alongside physicians, therapists, and hospital case managers. We know how discharge decisions get made because we've been in the room when they were.

Family-owned

You're not a case number

No shareholders, no volume targets, no rotating queue. We take on few families so yours gets someone who knows the situation without opening a file.

Independent by design. We don't own home care agencies and we don't take referral fees. When we recommend a provider, it's because we compared the options and that one was best for your parent.

How is Canyon Care different from other advocates or care services?

There are four other types of service in this space. Here's what they get wrong, and what we do instead.

  • Non-clinical care managers and concierge servicesAnyone can call themselves a care manager or patient advocate. No license required, and many have never worked in a hospital. Every Canyon Care advisor is a California licensed registered nurse. We read the chart, question the medication list, and catch the red flag before it becomes an ER visit.
  • Hourly advocatesAt $150 to $400 an hour, the meter is always running and families end up rationing their own calls. We charge a fixed price so cost never factors into whether you pick up the phone during a crisis.
  • Home care agenciesThey recommend their own staffing, because that is what they sell. We provide no home care at all. We research hundreds of services, filter to the top handful, gather quotes, and hand you the best one.
  • Medicare sponsored care coordinationReimbursement caps at roughly an hour a month, and there is little incentive to go beyond it. The program is built to ration your time, not to solve your problem.
Why would I need this?

Managing healthcare becomes a second job. Appointments, records, billing, and the constant low-grade worry that something is being missed.

We take that work off your plate and put a nurse's judgment behind every decision. Fewer late nights, fewer mistakes, more time for the parts of your life that matter.

Is this right for my situation?

If your health, or a loved one's, has more moving parts than you can track, yes. A new diagnosis. Multiple specialists who don't talk to each other. A hospital stay with no clear plan after. A chronic condition you suspect could be managed better.

If you're still unsure, book a call. We'll hear your specific situation and tell you what a solution would actually look like.

Do you replace my doctors, or provide home care?

Neither. We are the layer between them. You keep your own physicians, and we make sure they're talking to each other, that nothing gets lost between visits, and that someone with clinical training is reviewing the whole plan.

We do not staff home care, which is exactly why our recommendations are honest. When aides or home health are needed, we source around 15 quotes and hand you the best fit at the best value. We have no stake in which one you pick.

What if I'm not local?

Most of our work happens by phone, portal, and paperwork. Coordinating providers, chasing records, fixing billing, staying in contact with the care team. Distance doesn't change any of that.

The limit is in-person work. Outside our local area we can't do safety walkthroughs, and appointment attendance has to be virtual.

Does insurance or Medicare cover this?

No, and that's deliberate. Medicare's care coordination benefit pays for about an hour a month, which means a billing code decides how much help you get.

Working directly for you means no caps, no prior authorization, and no third party limiting our time. Flat monthly pricing instead of an hourly meter.

How do we get started?

Book a call. We'll talk through your situation and tell you honestly whether we can help.

If it's a fit, we lay out the next steps and start pulling records from your care providers to build your personal health system.

Still have a question?

Book a call and ask it directly. We'll tell you honestly whether we're the right people to help.
